Application Instructions

Cotillion for Achievement Scholarship applications are accepted each year during September and October. Applications may be obtained from your high school counselor, requested by email, or downloaded by clicking the A logo below.

Before completing the fillable application:

• Download the application to a desktop or laptop computer. Do not complete it in Google Drive or an internet browser preview.

• A paid Adobe Acrobat subscription is not required. Windows users may use the free Adobe Acrobat Reader. Mac users may use Preview or the free Adobe Acrobat Reader.

• Save the application to your computer using a new filename.

• Enter a small amount of information, save and close the file, and then reopen it to confirm that your information was retained. Save your work frequently.

• When finished, review every page, then print one complete application and check that all responses are visible.

• Obtain all required handwritten signatures.

• Give the completed, signed application to your school counselor according to your school’s instructions.

Cotillion for Achievement is a leadership development and scholarship program, focusing on developing leadership skills, honoring outstanding area high school seniors, and awarding them scholarships.
